Move More with Active SWV - INDIVIDUAL Promise Form

Active SWV, in partnership with the WV Physical Activity Plan is bringing you an important public awareness and behavior change campaign, "Move Your Body, Change Your Life."

By filing out this Move More Promise Form, I promise to:

  • Identify the ways I'm already moving throughout my day – from walking to the mailbox to vacuuming the living room.
  • Determine how many minutes or steps of various activities (ReFit, paddling, skating, pickleball, etc.) equate to a mile. (Active SWV will assist.)
  • Gradually add more movement to my daily routines, building towards the CDC-recommended 150 minutes of physical activity per week.

By filing out this Move More Promise Form, I promise to participate in the Move More with Active SWV Challenge! For the duration of the 6 week challenge (beginning when this promise form is submitted), I will:

  • Identify the ways I'm already moving throughout my day – from walking to the mailbox to vacuuming the living room.
  • Determine how many minutes or steps of various activities (ReFit, paddling, skating, pickleball, etc.) equate to a mile. (Active SWV will assist.)
  • Gradually add more movement to my daily routines, building towards the CDC-recommended 150 minutes of physical activity per week.
  • At the end of six weeks, I will fill out a quick email survey about my experience and Active SWV will mail me a free participation prize.
